This form consists of four sections: 1) Information about the Club / Society
Advisor, 2) Information about the Administrative and Supervisory Board
Members, 3) General Assembly documents including the Supervisory Board
Report, 4) Annual program of activities. As sections are completed, the
next section will open. You will have to upload the following documents:
a) The advisor’s acceptance letter, b) The list of attendees, c)
The minutes of the meeting, d) The report of the Supervisory Board, e)
The annual program of activities, and if changes have been made to the
Club/Society charter f) New version of the charter.
