An Activity Cancellation Form can only be submitted by a member of the Club / Society Administrative Board.


➤ GENERAL INFORMATION ABOUT THE ACTIVITY

If the activity will be held in Turkish, indicate the title both in Turkish and in English.

➤ DATE AND TIME OF THE ACTIVITY TO BE CANCELLED

Give the number of the official memo of the activity approval received from the Student Activities Center if known.


➤ SUBMISSION OF THE FORM

When you submit this Activity Cancellation Form, a summary will be sent to your email address and the Student Activities Center at the same time.
